bungwholes all day. Actually, most of the staring is at a screen. Still, whatever.
And I know, that cost is split between a doc, a nurse, and an anesthesiologist, each of whom spent maybe 5 minutes talking to me, then a 15 minute procedure.
The main reason I could never work in medicine is knowing that I'm getting like $50 an hour, for an hour that will be billed out at $10,000. It would be sooooo depressing busting your butt as a nurse (even a doctor) only to see a patient bill, a patient you spent maybe an hour or so working on, who was billed $237,400.

He retired around the time they started becoming your enemy, maybe around 1990. He said it was all going to go to crap, and he was right. His was a small community hospital, but he saw the writing on the wall. His hospital is now part of the giant McLeod conglomerate.
My personal physical just retired couple of years ago. He literally started a family practice, and was an EXCELLENT doctor. The overhead, and dealing with insurance companies drove him to eventually sell his practice to the Lexington Medical conglomerate. He would often miscode things just to get procedures that SHOULD be routine, covered. He told me I'd need a stress test every 5 years and put me on statins years ago, due to family history. He said every 10 years I'd need a nuclear stress test. When 12-15 years went by with no stress test, nuclear or otherwise, he kind of slumped his shoulders. He apologized but insurance won't cover it. I NEED one, in his medical opinion. He then asked me if I had any chest pains. I said I had some a few days ago, with a wink. Got the test done. He told me he's saved at least 5-10 lives going against the mandates of insurance companies. Once he sold out he told me the practice was going to go to crap. He was just staying on a few years for the "transition" and was retiring. And he did. 2 years later, the practice is now in a strip mall, and you can't even get an x-ray there anymore. I've changed doctors and practices now. Another guy, same history as I have, would not allow a stress test. He said he had chest pains (even though he didn't), doc got him a stress test, found a huge blockage, and was sent for emergency surgery. Saved his life. Right after telling me this he asked if I had any chest pains

0% chance the doctor made that much, you’re paying their fee, the anesthesiologist, and the facility fee. A standard colonoscopy is worth 3.26 RVUs, let’s assume your GI doc has a generous conversion factor of $60, and that’s a $195 pre tax earning. Now if he owns the building either himself or in a private group then he’s going to get a post-overhead piece of that facility fee, but that piece isn’t going to be as much as you think.
I think people grossly overestimate how much doctors are paid and underestimate the risk we take on every time we do something, even the smallest cases. What if you bleed from a polypectomy and have to go to the ER? What if he avulses the inferior pole of your spleen making the turn? What if you get a perforation? What if he misses something and you end up with a cancer that could have been caught early? What if he finds a cancer and has to do an EMR and puts the rest of his day an hour behind schedule? What if your wife says something condescending or ungrateful when he goes to talk to her afterwards?
When you consider all of that, $195 x .7 doesn’t sound like a whole lot of money. To make low RVU cases worthwhile you have to churn out volume, I bet he’ll do 10 scopes today.
