What's the deciding factor on who runs out first? Does it come down to who practices best this last week before the game?

Agreed. The OP is reading too much into the depth chart. There may be an isolated case where two guys are even then the position coach will make a game day position. In other cases like the OL where the two deep is listed but if a guy goes out there is often a shuffle so the best five are always on the field instead of the guy listed as 2nd string at the position.
