short yardage game (3rd or 4th down with < 2 yards to go). apparently we rank 45th in this dept. With that said, what formation/package play call would you go with?

That's tight end territory. Play action pass to a big receiver in the middle. It's money if you have a big guy with good hands.

It's often a place for a TE out route, but without a reliable TE, there are other ways. Perhaps the best way for us is to use a WR in the slot (Rodgers or Overton) and have them run an out, slant, drag, stop or look-in route. Both of these guys are good enough to get the job done. We can also use the same slot receiver with a 3-back behind and inside him with WRs on the same side - would create all sorts of havoc on the edge..... or set that strong look, make it look like a run and reverse the RB on a counter against the flow..... or throw a tackle eligible pass to the weak side..... or set strong right and leak the RB out to the weak side, chip the DE and throw to the RB over the head of the DE..... or run QB counter.
Perhaps the best way is to just get a good line push and shove the DL back, put them on their butts and let Etienne run over and through them.

inside the five yard line, as in the usc game. Do you still throw it, or etn, jumbo package or qb keeper? Don't recall TL ever doing a qb sneak.

Good thinking. Why don't we use the jumbo package on 3rd and 2 ?
Or get the lanky QB to walk up under center and slide into the first open gap ?

That is only for the running plays. Overall 3rd down efficiency we are 14th at .474
BTW OSU is 1st at .570
