How do you watch the game on ESPN3 if your local provider does not carry the service?

Gold membership, but your ISP still has to be an approved ISP. I think it most cases, ESPN3 is now available on all or most major ISP's.

a gold XBL membership does not give you ESPN3 access, it only allows you to use the app.

NO, football will still be on ESPN3, it just has to be an exclusive game meaning that the game cannot be broadcast on another ESPN platform. ESPN used to televise games on TV and also have them available on ESPN3, but they stopped that and moved those types of games to watchespn.com only.
