Than the 1st quarter shot at ETN after his helmet had already been ripped off. The face mask penalty was called, but hitting a player without a helmet is a bit thuggish.

Had the whistle blown? I dont know if it is automatic with a helmet coming off. That would stop a lot of plays.

So even if they did not call a penalty on the field, the central command center could have still called a penalty. I have a hard time with how they could have not seen the penalty as the play unfolded from the angles available on the command center monitors.
