For those that think we’re doing a better job than Italy

Mar 21, 2020, 11:18 AM

Numbers don’t lie. By next Sunday we will likely be where Italy is today

Re: For those that think we’re doing a better job than Italy

Mar 21, 2020, 11:28 AM

If we consider Italy has less than 20% the population of the United States (~60 million vs 330 millions), the numbers suggest we are doing a much better job

Re: For those that think we’re doing a better job than Italy

Mar 21, 2020, 11:33 AM

The gap will widen exponentially the next week.

I’m constantly surprised how many people don’t understand exponential modeling.

Y’all know that US had the worst testing capabilities per capita in developed countries, right? Numbers are about to explode now that we’re up to a respectable number of tests as of yesterday

There’s also a TON of people that are asymptotic

Mar 21, 2020, 11:37 AM

That numbers will never show. Get off your numbers high horse, bud!!!!! Data is not accurate. No point in speculating it one way or the other. Amazing how many people think they are experts just by reviewing what others are showing on a small sample size. There is nothing accurate right now. Just keep freaking out and the rest of us will stay calm and do our “part”.

Quite the opposite, I’m one of the few people that hasn’t had my incomes interrupted at all. I’m also young and healthy. I also got my PhD from Clemson in 2012 and am embarrassed, but not completely surprised, by the lack of intelligence by many on these boards.

It’s not that people are asymptomatic, it’s that the incubation period is so long they haven’t started showing symptoms YET. Big difference.

When you contract the flu virus and you haven’t gotten sick yet, you don’t tell people you are asymptomatic

Assuming exponential growth Goes on indefinitely

Mar 21, 2020, 11:55 AM [ in reply to Re: For those that think we’re doing a better job than Italy ]

Is incorrect. None of the numbers in any country have continued along the exponential route for very long. If they did Italy would be in the astronomical numbers by now since they are a decent amount of days ahead of us.

I’ve seen several experts on twitter recently pushing back against the indefinite exponential assumptions.

Also as far as our daily lives what do the numbers really matter? The important thing is that the hospitals have everything they need and we just stay home and don’t contribute to the problem.

I am constantly surprised how many people do not understand the more cases that exist and are asymptomatic or negligible symptoms with no treatment needed, the more it proves the point that they are fear mongering and do not even understand the virus at the most basic level and actually enjoy pushing fear into others.

This would mean the coronavirus is far more common and far less deadly than presently thought. And that a certain amount of herd immunity exists in the country. The data from other countries doesn’t support the idea it is going to kill millions. Figuring out how many people in the American public, both symptomatic & asymptomatic, have the coronavirus is massively important because we can't make smart public policy without it. Your comments and posts are just really poorly thought out and not accurate. Read the data, "read a book".

I believe it's been here longer than we realize. There's just been no testing. I've had employee's, family and friends who've been been sick for weeks. Fever, breathing problems, body aches, dry coughs and more. Repeatedly they'd be tested for for the flu and the results would comeback negative.

The really confusing thing was they'd improve and then relapse. What a strange time for us all...

The virus is not alive....viruses are not living organisms...never confuse opinions with Scientific Method...they don't work together! The epidemiology of this disease is fixed by different living and associations of people in different countries...Italy is completely different from the United States...China is radically different from both! Vectors, transmission, infection and cure rates depend on many variables that we don't have a handle on yet! For sure as stated below the China press and medical personnel were restricted in the ability to report this disease...cases might have occurred all the way back to September! When this is fixed and we have a good timeline we can look back and make the right corrections...all you Trump haters thank your lucky stars that we have a real president instead of a community organizer (almost lawyer) with microencephalopathy! Go look at a google earth satellite view of Wuhan...about 200 miles west of Shanghai...if you were looking to spread a disease...sprawling cities of China would be a great place to start!

“The infection date is climbing faster because we’re testing better”. Lol, no the infection rate was already climbing fast, we’re just now able to track it slightly better than a week ago when no one could get a test other than NBA players.

True Italy’s mortality rate is high because they have a higher % of >65 citizens due to decreasing birth rate. That and they all live with their families instead of nursing homes so they caught it faster than our older citizens have.

At this rate we should be passing China’s infected number in ~9-12 days

First, Italy has a mortality rate of 8.6% (4032 divided by 47,021). Second, the reason that the infection rate is climbing in the U.S. is that more people are being infected. We are waaay behind the curb on this thing. We have no idea how many infected there are. But we do know that however many there are, they're infecting others.

This thing could get way, way out of hand. There is no excuse for not having millions of test kits in the U.S. by now.

Another huge factor is our health care system is a yellofa lot better than Italy's.

Our healthcare system ranks 37th in the world, Italy's ranked #2. The myth that the U.S. has the best healthcare system is one perpetrated by the U.S. Healthcare system. The World Health Organization has us ranked #37 as well.

Italy is no where near as ready for this according to the New York Times they show Italy with 3000 ventilators one for every 20000 citizens We have 160000 or one for every 2000 citizens. Overall ratings don’t mean crap.

Keep reading that we are ten days behind Italy. US death rate is currently and thankfully less than a tenth of what the Italian death rate was ten days ago... But you keep fighting whatever fight you are trying to and trying to scare people for whatever reason... I know, numbers are hard :)

flag link military_tech thumb_downthumb_up

Re: Well duh . . .

Mar 21, 2020, 11:54 AM

Italy’s death rate grew when they ran out of ventilators and had to start making choices on who to keep alive. Our hospitals will have similar choices soon based on recent numbers and hospital reports.

Again, death rate isn’t everything. If you get in a car accident and lose your legs, do you come back to work on Monday and say you’re fine?

Likewise, if you get hospitalized (current rate is 20% across all ages) get scar tissue on your lungs, incur a $5-10,000 bill (assuming insured), and then go back to your lack of a job. That’s good?

Italy was much healthier than the US

Mar 21, 2020, 12:54 PM [ in reply to Re: Well duh . . . ]

That’s why they live five years longer. We already had epidemics of obesity, diabetes, and heart disease that other countries don’t experience.

But he’s right. In Italy when they ran low on ventilators, patients over 80 didn’t receive them. As their cases rise, that age threshold is being lowered so the patients with the best chance of recovery get priority.

Mar 21, 2020, 12:45 PM [ in reply to Well duh . . . ]

Exponential model, in kindergarten terms:

Patient 0 = 1 patient
Patient 0 spreads infection to 1 over some period = 2 patients
Those 2 spread to 1 each = 4 patients
etc.

If people didn’t move around the country and world so easily, the total population wouldn’t affect the rate at all.

flag link military_tech thumb_downthumb_up

Re: No, that not how it works

Mar 21, 2020, 2:24 PM

And in 33 steps the infected population is more than the number of people in the world. It can't go on indefinitely.

One of the big questions is "What is the real numerator" in these ratios? With the apparently large number of asymptomatic people, the death rate per infections can only be estimated. Death rate per confirmed infection is larger number.

2024 white level memberbadge-donor-20yr.jpg flag link military_tech thumb_downthumb_up

Re: No, that not how it works

Mar 21, 2020, 8:02 PM

Clearly it won't go on forever. But does it HAVE to in order for this to be serious? This isn't going to grow exponentially until 300M people are infected. But if it grows exponentially until 0.3% of our population (1M) are infected, then our medical system would still be wrecked with 10s of thousands likely dead before Fall.

flag link military_tech thumb_downthumb_up

Of course not

Mar 21, 2020, 9:44 PM [ in reply to Re: No, that not how it works ]

Even if everyone eventually became infected, the rate couldn't continue the whole way with the same time constant. Later in the process, an infected patient may encounter the same number of people, but doesn't infect them all because some are already infected, recovered, or resistant.

Nobody argues that the infection grows exponentially forever. You are just missing the point.

We already had a ton infected but our testing bandwidth of now 40k per day is making our numbers go up. Testing is showing that a ton of people are infected but the death rate is slowing down. I bet way more than 47k in Italy are infected. Same with China, same with us. Look at Germany with 21k cases but a death rate under 0.5 percent.

The increase in US infections was/is expected but unlike Italy, we are also seeing a corresponding drop in our mortality rate (we started at 2.0% last week and have dropped to 1.3% as of today). Who knows, we could see our mortality rate drop to below 1% as we identify more infections. Bottom line is that Italy's mortality rate has steadily increased with their infections. We are two different countries with different cultures, different medical capabilities etc... So I would not use Italy as some baseline for what is going to happen in the USA.

Finally, not to sound crass, but I don't care what any other country's COVID-19 numbers are. The ONLY numbers that matter to me are the USA numbers because it is the only numbers I ultimately trust. Let's all take a breath and see where we are at at the end of next week...

Wouldn’t the death rate not be accurate until the patient has actually recovered? If 5,000 cases were added at this moment then the death rate would go down right away. However, that’s assuming that none of those 5,000 would end up dying from the virus before recovering.

It's all a snapshot in time - the mortality rate is just as accurate as the recovery rate at a specific moment in time. If you ain't dead you get counted as living - simple as that. Of note, Italy has never had a drop in their death rate it has done nothing but steadily climb. The USA is on an opposite trend - as we have confirmed more infections our mortality rate is actually dropping - let's all pray that continues.

Don't try to argue math with them. They think that because we are finally able to start testing and have gotten 18,000 new cases in the last 3 days, that the corresponding "drop in mortality rate" is a good thing. As if that means anything.

Patients are dying 1-2 weeks after contracting the virus, not instantaneously. The death rate lags behind the infected rate, so it will be lower than the true value until the infected rate levels off from exponential growth. At that point, mortality rate will increase until it too levels off and we have a true value.

It was hyped in a Fox News piece that Trump saw and he ran with before asking anyone with real knowledge. Most of the Fox News types saw Trump hype it, then completely missed the expert correct him seconds later on the same stage and the FDA correct him minutes later in a press release.

Some antivirals are being studied by the NIH but those well controlled clinical trials will be finished about the time everyone in NY is dead! Scientific method sometimes must be thrown out the window...the Quinines are safe...especially the newer synthetic ones...and Losartan has been used effectively for BP for 20+ years and is void of major side effects...we will know soon and we'll get over this one! Israel is letting the younger healthy folks get subclinical cases and protecting the compromised to allow 'immunity'...it will be a long time before a vaccine is available...if you've cleared the virus you are probably already immune! Before I thought you were talking about exponential growth. But now it seems you are just confirming that we have a larger population. Correct me if I misunderstand you.

Re: So basically you are now saying that we have a larger population?

Mar 24, 2020, 10:34 AM

Both. Exponential growth was guaranteed, as it's present in every country. But many on here misinterpreted my original post to say that I thought we'd have the same number of cases, and thus that isn't a big deal because we have 5x the population.

Clearly we were always going to have way more cases. We're ~1.8x total cases of Italy at same timepoint (Day 21), and would not be surprised if it was 3-4x by Day 28. It's very conceivable that we'll have more cases (per capita) than Italy before the end of next week.

I think many posters on here need to relearn what "exponential" means. Exponential doesn't mean it doubles daily, it just means that the growth rate is increasing at a constant rate.

If you have 10 new cases every day, that's not exponential, that's linear. If you have 10 new cases, then 20, 30, 40, 50, 60 in successive days--that's exponential. That's what we have. Every single day for the last 10+ days, there has been more positive cases than the day before. That is exponential growth, and it will likely last for several weeks--longer if Trump decides that we can all go back to normal lives on April 1st like he wants.

flag link military_tech thumb_downthumb_up

I get you but it can only maintain that same degree for a time.

Mar 24, 2020, 11:00 AM

It sounds like you are more the mathematician than I am. But eventually you have to slow because of the population. Obviously you can’t reach a billion cases in America. What I am interested in mathematically regarding this follows. And I’m not sure it’s easy to model. Penny for your thoughts:

So, we are flattening the curve. I get it. Trying to not overload the surge capacity of our medical system. In other words unlike Italy, we would ideally like to not have more patients that need ventilators than number of ventilators we have. This is especially difficult in population dense areas like New York City where we are already on the brink of this. Mathematically, will we end up with the same “area under the curve” in the long run? Which I assume would be patients. So in other words will we end up with the same amount of people getting the virus just that we are hoping to have enough ventilators and therefore the illness drags out. I know your intent seems to be to make this political. But if this is exponential growth it is difficult to exponentially grow your resources when your resources have been set over time. For instance our medical system is not built to maintain a pandemic all the time. Many hospitals have cut beds, staff, facilities, etc over the last 20 years, why, because it’s not profitable. This is not the fault of one president. We are built for the non-pandemic times. Could we have acted more quickly, of course. Could we have undone the trends of our hospitals over the last few decades in a few weeks? No. Could we have “exponentially” increased our number of ventilators (which is essentially what we are talking about here). No. Thoughts? Less interested in your political arguments, but your mathematical.

Re: I get you but it can only maintain that same degree for a time.

Mar 24, 2020, 11:57 AM

Absolutely it will slow, but there's no reason to think it will slow for quite a while. We have a population of 330M, even if 60M get infected (which would be catastrophic as we're talking 600k-1M deaths), that's still less than 20% of the population being exposed, which still leaves ability for the virus to move around. Social distancing will help, but it's economically unrealistic to expect us to have Shelter-in-place for months, which is really what it would take to slow this.

From what I understand, you're correct in that "flattening the curve" does not inherently lower the Area under the curve, it just lowers the peak (which is the dangerous part for the reasons you laid out).

If we were to not practice social distancing at all, the good news is that the Pandemic would be over sooner. The bad news is that the mortality rate would skyrocket into the double digits, and our entire infrastructure would crumble as millions of people require hospitalization at the same time. The good part about flattening the curve is that we can try to keep the number of hospitalizations manageable at one time, but it is going to significantly lengthen the overall time that this is a Pandemic--which is not necessarily bad as it will give our Pharmaceuticals a chance to develop anti-virals and Vaccines.

What will be interesting to watch is when do people start getting impatient and going back to "normal" lives. Trump is already suggesting that he will want to remove restrictions next week so that the Economy can get started again.

Many people in areas not under siege (rural areas in the south and midwest, primarily), don't see this as a big issue because they see their low numbers. The problem is, by the time any of them think it's a big deal, it will be too late.

Additionally, while dense cities will predictably take the brunt of this early on, they are the most equipped to handle it in terms of resources and, statistically, their population is younger and healthier than rural America.

Many don't realize that >50% of counties in America do not have a single Acute Care bed. While Sar-Cov-2 will spread to rural America much slower, it is very likely to have a much higher mortality rate due to the demographics of those areas.

Good arguments. Some will remain to be seen. Live in Arkansas.

Mar 24, 2020, 12:06 PM

We are considered backwards by many standards. But I’m convinced, without the data to back it up, that we have more ventilators and Hospital beds per capita than New York City. We have 200 cases now, seven on ventilators, no deaths. Almost 800 negative tests. The deaths will come but I like the negative tests. Maybe that means we we’re testing sooner than most relatively speaking.

Re: For those that think we’re doing a better job than Italy

May 5, 2020, 1:03 PM [ in reply to Re: For those that think we’re doing a better job than Italy ]

I guess it depends on the definition of "bad".
1. We really should be talking per capita numbers.
2. As time goes on, the rate at which we measure people will increase, so that contributes to the exponential curve.
3. Since we have so many confounding variables in cases, it seems we should look at deaths. It isn't hard to measure if someone is dead. Of course, this brings up its own issues, such as the age of the population and other factors like hospital availability, which brings us to #4...
4. As I understand it, all of these mitigation strategies are designed to prevent our hospitals from getting saturated. So maybe free hospital beds is the metric to follow.

Having said all of that, I do think it is a crime that our testing is so far behind where it should be and where we were told it would be. If we had more testing, we may not be in lockdown now. Lack of leadership didn't cause the pandemic, but I do think it has dug a deeper economic hole. Of course, it was nice to see Congress send money to big corps with deep pockets that it said was intended for small businesses. Disfunction abounds.

May 5, 2020, 3:05 PM

Not disagreeing with the content of your post, and I appreciate your fairly balanced approach when you touched on the political aspects.

I just want to point out that the criticism of 'money intended for PPP loans went to big corporations' is typical incorrect, shortsighted, unresearched mudslinging journalism that blames the wrong people.

1) The SBA administered the loans.

2) The SBA assists companies from 1-500 employees. Many major corporations have smaller companies that comprise the corporate family. Some of those smaller companies fit the criteria to be served by the SBA. The LA Lakers may have a billion-dollar payroll (I don't know), but not be disqualified from a PPP loan due to the economic hardship of the pandemic. The Lakers returned their money because of either PR reasons or reason #3 or #4, not because they weren't eligible (if they have <500 employees.)

3) The SBA normally loans $1B per year. They require reams of professionally prepared paperwork to get a loan. Under PPP they were asked to loan over $600 B over two waves within 3 weeks. In order for that to be possible, both the SBA and lending banks were excused from culpability for fraudulent applications. According to the terms of the loan, companies applying for the loans inappropriately bear the responsibility for their fraud.

4) Also according to the terms of the loan, companies that applied for PPP had to swear that no other emergency funding program could help them survive. But the first-come, first-served nature of PPP necessitated apply-first, then check out the other programs. When companies found other sources of emergency funding they were free--and expected to--return the PPP money.

5) Financial advisors were recommending to their clients to apply for PPP whether the funds were needed or not, without regard to the application details. A 1% loan that becomes forgivable?? What's not to love! Except it's not for your clients, Smith Barney.

6) The criticism of the program involved (by my understanding) less than $300 M of the more than $600 B loaned. That's 1/2000th of the total funding. That assumes that 99.95% was loaned correctly.

As I said, the criticism is valid if it is aimed at the correct people, in correct perspective, with an understanding of the terms of the program and the strategies these companies were advised to follow.
