Ma belle femme wants to run a 5k (Because females are illogical). Last summer she started a couch to 5k program and ended up having to stop due to tendonitis in her knee. She started again this year and again has tendonitis. Last year she received physical therapy and followed it and it went away. She can walk long distances and can hike up and down mountains with no issues, but as soon as she runs she gets tendonitis.

Suggestions?

that way we can see her knee better and can better troubleshoot the issue. That and a white tshirt with no bra that has been drenched in water within the past few seconds.

There are a few options

-Obviously shoes (even though you already said you tried this)

-Shorter strides on her end. Shorter strides increase your ability to "land" properly. She should be landing forefoot to midfoot. If she is a heel striker, this will more than likely lead to injury.

-Run on trails or softer surfaces. Concrete and pavement can be a #####. I even notice more pain in my legs when I run on the road.

-Knee support brace

-Cut her legs off at the knee, buy her a wheelchair. Knee pain COMPLETELY eliminated.

This ^^^

Stride/striking makes a lot of difference.

If she can't contact an area coach, I would suggest incorporating lots of sprint work into her training.
About impossible to run sprints and land on your heels.

Lean forward a tad as she runs which helps direct landing on fore foot vs heel.

It's almost like learning to run all over again.

Don't pump arms to the chest... let them flow in a relaxed fashion around the waist (unless sprinting ). Takes drive away from legs.

Lastly, IF NECESSARY , take 1 advil before and 1 after run.

Sounds like she needs to change her running style. Sprint work, shorter strides, lean forward will help her endurance, speed and form.

in your area? There are a few here that use sensors and film you running to see what your pronation is and can can recommend shoes/insoles to counteract it

Get minimalist shoes. No cushion and 3mm drop or less. Run on trails. Start at 0.25 runs and go from there.

A lot of it has to do with cadence and where she is striking. If her cadence is around 150 or so and she is heel striking hard, she is over-striding and putting a ton of pressure on her knees and her hips. I would suggest as Greenr did a GOOD pair of shoes (from a shop like Fleet Feet or a local shop that will evaluate her stride and strike) and someone who can help her chop down her stride and get her cadence higher. I would also look into therapeutic gel packs to help her after each run, get the one that you can heat and cool.

Some of the tendonitis could be from her IT band being extremely tight. When I get to a better connection I will t-mail you some ideas for stretching.

Tight IT bands cause this and if her issue is on the outside of the knee, I bet rolling them will help. I would get a black foam roller because it is firmer than the white. Good luck.
