I also follow tier replies easiest looking at the forum indents. Seems harder to determine who relied too who looking at thread indents.

Just had one thread with many ads. Something loaded in that div block obviously. I've seen some ad setup sites that can run your site code, and gives back all ad placement potentials. Seems like it captured that avatar block.

Looks like you're trying to place ads in between replies (top of attached), but it's still jumping into avatar div block (lower area of attached).
Seems like you could add a dynamic div block to appear if reply exists, by number of reply. So, something like:
If reply 1
No footer div
If reply 2
No footer div
If reply 3
No footer div
If reply 4
No footer div
If reply 5
Add footer div, name div
Add advert in footer div
If reply 6
No footer div
.......
Repeat to include div block every 5 replies (or whatever interval)
Similar to this demo, where the "add element" is identified and executed by the above logic instead of manual clicking to add.
Just a thought.
