It means that the player is healthy and eligible to play, but the coach has decided not to use him. Players usually become a healthy scratch after poor performances or as a wake-up call.

1. He was getting half his snaps as a DT which isn't putting him in a position to be successful. He was a Top 10 run stopper and top 15 PFF pass rusher despite his sacks not showing it...and he's only going into year 3 pass rushers usually wake up during year 3 and now they've replaced him with ngakue.
