as it bounces up and down during high traffic times. I see all of the missing chunks of concrete and rust when I go under bridges on the lake, it doesn't inspire confidence in our infrastructure. Maybe those bridges are just getting broken in when the rebar becomes exposed, I dunno.

exposed rebar is always bad

when steel rusts it grows by adding oxygen to make iron oxide. It gets physically larger and heavier on a molecular level. When the rust grows it breaks off the concrete by spalling. The more spalling the more exposure the more corrosion. The steel losings its tensile strength at the same time and sooner or later you will have failure, almost 100% of the time.
